Services

The steps that are involved in The Path Service Brokerage are:
Planning
The Path Service Brokers will act as facilitator in a person centred planning process to help the individual, together with their circle of family and friends, identify what their priorities are. The plan would cover long-term as well as immediate requirements. It is essential that some sort of plan be developed before a package can be constructed.
Advice
The Path Service Brokers provide advice on what services and resources are available to meet the individuals’ identified needs. This will include accommodation, social/recreation and employment/education options.
Development of Services
The Path Service Brokers will negotiate with service providers to develop new services or adapt current services to meet the client’s needs.
Negotiations
The Path Service Brokers will negotiate the best service for each client; maximising choice and minimising cost for each element of the service package. The Broker will then develop a contract setting out what is to be delivered, when it is going to be delivered, at what cost and will Include review mechanisms.
Monitoring and review
The Path Service Brokers will monitor the performance of service delivery. Assessment will be against the contract requirements and the Disability Standards. If services are not meeting the contract requirements or the Disability Standards the DIRC Service Brokers may terminate the contract and renegotiate with other service providers.

The Path Service Brokers will renegotiate contracts when there is a change to the client’s circumstance and the change will benefit the client.
Documentation
The Path Service Brokers will maintain a client file. The client will have access to these files when and if they request. The files will detail the person centred plan, the process of developing that plan, the services that have been implemented, the contracts that are in place and the review of these contracts.
